OhmyOlivia Telehealth Consent

By clicking “I Agree,” checking a related box, using any other acceptance protocol presented through the service, or otherwise affirmatively accepting this consent, you confirm that you have read, understood, and agreed to this consent. If you do not agree, please do not create an account or use the service. You also authorize anyone who clicks “I Agree” on your behalf to do so.

Purpose

This consent form (“Consent”) provides information about telehealth and seeks your informed consent to use telehealth for healthcare services provided by physicians, physician assistants, and nurse practitioners (“Providers”) through OhmyOlivia’s online platform (“Service”). The terms “you” and “yours” refer to the person using the Service.


Use of Telehealth

Telehealth delivers healthcare services via electronic communications, information technology, or other means when the patient and provider are not in the same location. Telehealth can be used for diagnosis, treatment, follow-up, and patient education. This may include:

  • Electronic transmission of medical records, images, personal health information, or other data.
  • Interactions via audio, video, and data communications (e.g., messaging or email).
  • Use of output data from medical devices, sound, and video files.

Alternative care methods, such as in-person services, may be available and can be chosen at any time. Discuss these options with your Provider.


Anticipated Benefits

The potential benefits of telehealth include:

  • Easier and more efficient access to medical care and services.
  • Convenience of obtaining care at times that suit you.
  • Ability to interact with Providers without the need for an in-office appointment.

Potential Risks

While telehealth can offer benefits, there are potential risks, including:

  • Limited quality, accuracy, or effectiveness of services.
  • Technology issues such as bugs or errors that may limit functionality or produce erroneous results.
  • Failures in technology that could affect your Provider’s ability to diagnose or treat.
  • Inability to conduct certain tests or assess vital signs in person, which may prevent proper diagnosis or treatment.
  • Possible requirement to seek alternative or emergency care if telehealth services are not suitable.
  • Delays in treatment due to technology or Provider unavailability.
  • Potential breaches of privacy due to electronic system failures.
  • Unintended disclosure of health information through electronic communications.
  • Regulatory limitations on certain prescriptions or treatments.
  • Lack of access to all medical records, which could result in adverse drug interactions or judgment errors.

Life-Threatening and Emergency Situations; Follow-Up Care

For life-threatening emergencies, such as contemplating suicide, call 911 or the 988 Suicide & Crisis Lifeline at 988.

For urgent care, telehealth may not be appropriate. Seek care at an emergency room or other urgent care facility. Providers may not respond promptly to communications through the Service. For non-urgent situations, use the secure message service within the platform.


Data Privacy and Protection

The Service uses network and software security protocols to protect your information, including measures to safeguard against corruption. Your personal and protected health information will not be disclosed without your consent, except as required by law. This includes situations like danger to self or others and mandatory reporting of abuse. Note that email communications may include your protected health information and are subject to security limitations.


Your Acknowledgments

By clicking “I Agree” or otherwise accepting this consent, you acknowledge:

  • Healthcare services via the Service will be provided through telehealth.
  • Your Provider may be a nurse practitioner or physician assistant, not always a physician.
  • Certain technology, including the Service, may be in beta testing, potentially limiting functionality and accuracy.
  • Telehealth technology may involve uses not described in this Consent.
  • No guarantees of specific results from telehealth or related laboratory testing.
  • Telehealth may not be suitable for every condition and may require alternative or emergency care.
  • There are risks associated with telehealth, as outlined in this Consent.
  • You can discuss telehealth benefits, risks, and alternatives with your Provider.
  • No recordings of online sessions will be made by you or your Provider.
  • Providers will assess your condition and determine the appropriateness of telehealth.
  • You can withdraw your consent for telehealth at any time, but Providers do not offer in-person treatment. Withdrawals are effective upon written notice and do not affect actions taken before receipt of notice.
  • Electronic communication of personal medical information is part of the Service.
  • You must provide accurate and complete information to Good Life Meds and your Providers, including emergency contact details.

Pharmacy Orders and Prescriptions

OhmyOlivia partners with pharmacies to fulfill orders. You can choose any pharmacy for your prescription by contacting support. Prescriptions can be filled and transferred between partner pharmacies. You must cover the full cost of the Service, including prescriptions, and will not submit claims to Medicare, federal payors, or insurers.
